4. Categories of Records Held
A. Available without a formal request:
Practice location, services offered, and general fee structures publicly available on the website.
B. Available upon formal request (subject to strict POPIA and HPCSA confidentiality rules):
Patient Records: Intake forms, clinical notes, psychological assessment reports, and billing histories. (Note: Patient records are highly confidential and will only be released to the patient themselves or a legally mandated representative).
Financial Records: Invoices, receipts, tax records, and medical aid claim submissions.
Operational Records: Business licenses, HPCSA registration certificates, and practice management documents.
5. Process for Requesting Access to Records
To request access to records held by this practice, you must complete the prescribed Form 2 (Request for Access to Record), which is available on the Information Regulator's website.
Submit the completed form to the Information Officer via the email address listed in Section 1.
Provide sufficient detail on the form to enable the Information Officer to clearly identify the requested record and the requester.
Access to certain mental health records may be restricted, delayed, or require a mandated healthcare representative if the release of such raw data or clinical notes could cause serious harm to the patient's physical or mental well-being.
6. Prescribed Fees
A prescribed request fee may be payable before your request is processed, as gazetted by the Information Regulator. You will be notified of any applicable fees upon submission of your request.
